Complete your cat’s history
Discuss your answers with the whole household. Include as much detail as possible about your cat’s health, routines and the behaviour you have noticed, and mention where your accounts or views differ.

Arrange your veterinary referral
Each pet needs a veterinary referral and full medical history. You or the team can request these from your vet.
Send the questionnaire and a floor plan
Email your completed questionnaire to admin@petbc.co.uk before booking. Include a simple drawing of your home showing food, water, litter trays, resting places and other resources your cat uses. A line drawing is enough.
If you have video footage, please don’t set up or provoke a problem to film it. If particular footage is needed, we will explain how to obtain it appropriately.
