Dog pre-consultation questionnaire
A few things to prepare so we can understand your dog’s history and make the most of your consultation.
Complete your dog’s history
Tell us about your dog’s health, routines and the behaviour you would like help with. Give as much detail as you can, and discuss your answers with everyone at home. Please mention where your accounts or views differ.
The online form opens securely in Jotform. Download the Word version directly.
Arrange your veterinary referral
Ask your vet to send a referral and your dog’s full clinical history. We need this alongside the questionnaire before we can arrange the appointment.
Send your completed information
The online questionnaire is submitted through the secure form. If you have used the Word version, email the completed document to admin@petbc.co.uk.
The team can arrange your appointment once the questionnaire and veterinary information have arrived. Contact us if you are unsure what is still needed.
